Home Care Speech Language Pathologist, Chenango County Region

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients' homes, improving communication, cognition, and swallowing for safety and independence. Conduct assessments, develop evidence-based treatment plans, and adjust strategies based on patient progress. Document interventions and progress, ensuring compliance with regulations and reimbursement requirements. Educate patients and caregivers on communication and swallowing safety. Collaborate with interdisciplinary teams, participate in care planning, and recommend assistive devices. Maintain licensure, stay current with clinical practices, and mentor peers if needed. Qualifications include a Master’s degree, CCC or CFY, NYS licensure, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
