Tryfacta is seeking a LPN - LTC for our client in Grand Island, NE 68803. This is a 13-week contract assignment. Position Title: LPN - LTC Location: Grand Island, NE 68803 Duration: 13 weeks (Possible extension) Work Schedule: 12 hrs. (Day) Guaranteed Hours: 36/wk. Pay rate: Local - $42/hr. and Traveler: $47/hr. ($1692/wk.) Orientation: 8:45 AM regardless of assigned shift. OT may be available but is not guaranteed and may be offered on short notice. 36-hour weekly guarantee. Candidates may pick up additional OT shifts when available

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to LTC residents.
• Administer medications and treatments.
• Monitor residents and report changes in condition.
• Assist with patient assessments and care plans.
• Maintain accurate clinical documentation.
• Collaborate with CNAs, RNs, and other healthcare staff.
• Provide compassionate, patient-centered care.
• Support the team with flexibility when staffing needs change

To be considered for this position, you should have:

• Active LPN license.
• LTC/SNF experience preferred.
• IV certification – indicate on resume if certified.
• Strong clinical and communication skills.
• Compassionate and dependable with a strong work history.
• Every other weekend is mandatory.
• Must be flexible with scheduling.
• Block scheduling may be requested but is not guaranteed.
