Job Title: RN Case Manager – Floating Territories | Explore the Metro, 1:1 Care + Incentives

Company: Home Health Care, Inc.

Compensation: $95,000 – $130,000 per year plus incentive visits

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time with On-Call Rotation

As a Floating RN Case Manager, your day is spent on the road, in different communities, meeting new patients, and supporting different care teams .

What You’ll Do

• Travel across the metro to support different territories and care teams .

• Provide skilled nursing visits, assessments, and care planning .

• Complete OASIS visits and coordinate patient care .

• Build trust quickly with patients and families.

• Help maintain consistent care during growth or staffing changes .

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• Comfortable working independently in a mobile, field-based role .

• Strong clinical judgment and adaptability.

• Home health experience preferred, but not required.

• Reliable transportation and willingness to travel.
