Responsibilities
- Provide direct and indirect skilled nursing care to residents on the assigned unit under RN supervision.
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve the resident/family and healthcare team.
- Recommend revisions to the care plan after evaluating resident outcomes and coordinate referrals as needed to address all aspects of care.
- Collaborate with residents/families and Interdisciplinary Team to ensure safety, quality of care, and customer satisfaction.
- Maintain current nursing knowledge and competence; participate in mentoring/orientation of staff; demonstrate leadership under the direction of the RN.

Requirements
- Active New York LPN license
- BLS certification (AHA or RQI accepted)
- Minimum 1 year of LPN experience in a long-term care facility
- Ability to work the assigned shift (2x8 nights) and comply with every-other-weekend/evening/Holiday requirements
- Demonstrated ability to provide geriatric-focused nursing care and collaborate with an interdisciplinary team
- Effective communication skills and ability to document and report resident status accurately
