Physical Therapist-Rehab Services Mobile Lake Forest Full Time Days

Salary range: $41.00 – $53.30 hourly. Placement is determined by experience, internal equity, and, for union‑represented positions, the collective bargaining agreement. Job Summary Physical Therapist for the Mobile Therapy Team. Deliver outpatient physical therapy in patients’ homes and via telehealth, and support the Hospital at Home initiative. Use unit resources efficiently. Manage work schedule and complete tasks on time. Contribute to continuous improvement initiatives. Participate in cost‑saving and process‑streamlining efforts. Apply creative problem‑solving and adapt treatment to special circumstances. Select appropriate evaluations and treatment modalities; perform re‑evaluations per guidelines. Document progress notes accurately in EMR. Provide educational materials tailored to patient understanding. Participate in committees, department processes, and special projects. Build professional relationships with the healthcare team. Create respectful communication culture and respond to feedback. Complete non‑patient tasks on time (coverage notes, PTO/schedule changes, bonus pay forms). Participate in departmental educational efforts (in‑services, journal clubs). Serve as clinical instructor, mentor, or observation host for student programs. Meet productivity standards. Flex schedule or assignments to meet patient, staff, and department needs. Required Qualifications Bachelor’s Degree in Physical Therapy. Licensed Physical Therapist in Illinois. BLS CPR certification from American Heart Association upon hire. Preferred Qualifications Master’s or Doctoral Degree in Physical Therapy.
