Job Title: Transition Services Supervisor (TSS)

About The New Foster Care

The New Foster Care’s (TNFC) is a 501(c)(3) non-profit organization based out of Oakland County. Founded in 2016, TNFC is a faith-based organization committed to making transformative changes within the Michigan foster care system creating a measurable, and meaningful impact in the lives of youth in and aging out of foster care.

TNFC serves people ages 14-29 through its Bridge Program, a comprehensive transitional support model that centers the young person as the driver of their own plan. Participants work with a Program Supervisor, Peer Support Specialist, Personal Wellness Coach, and Attorney to address any issues hindering their progress and develop a strategy for success across six life domains: Education, Employment, Housing, Transportation, Mental & Physical Health, and Community Connection.

Our work is grounded in relationship-based, trauma-informed, and strengths-focused practice,delivered through an integrated multidisciplinary team.

Position Summary

The Transition Services Supervisor (TSS) provides day-to-day leadership, supervision, and professional development for our Transition Navigator team while ensuring high-quality, participant-centered case management.

In this role, you will coach and mentor staff to align with TNFC’s mission, organizational standards, contractual requirements, and the Transition to Independence (TIP) model. You will also maintain a small caseload (5–10 participants) to stay grounded in direct service, model best practices, and handle complex situations hands-on.

Key Responsibilities

Leadership & Staff Supervision

• Provide day-to-day coaching, supervision, and mentoring to Transition Navigators.
• Lead individual supervision and team meetings to review participant progress, service delivery, and documentation.
• Oversee caseload distribution to ensure equitable assignments and high-quality care.
• Support onboarding, recruitment, interviewing, and ongoing training for new team members.
• Foster a collaborative, trauma-informed, accountable, and servant-leader team culture.

Program Oversight & Service Coordination

• Ensure service fidelity to the Transition to Independence (TIP) Model.
• Monitor participant progress across all six transition domains.
• Maintain a small caseload of 5–10 participants to model participant engagement and best practices.
• Provide crisis intervention guidance, complex case consultation, and community resource coordination.
• Collaborate closely with Peer Support Specialists, Wellness Coaches, Clinical Services, and Legal Advocates.

Case Management & Compliance

• Develop and maintain individualized service plans in alignment with TIP model guidelines.
• Conduct participant meetings across community, home, and office settings.
• Review staff documentation for quality assurance, timeliness, and HIPAA/contractual compliance.
• Support staff with crisis response, risk assessment, and post-incident debriefing.

Community & Leadership

• Build strong partnerships with Metro Detroit child welfare agencies, referral partners, and community resources.
• Participate in program development, leadership meetings, and quality improvement initiatives.

Qualifications & Requirements

Minimum Qualifications:

• Education : Bachelor’s degree in Social Work (BSW), Psychology, Human Services, or a related field.
• Experience : Minimum 3 years working with youth/emerging adults in foster care, child welfare, juvenile justice, or behavioral health.
• Leadership : Minimum 3 to 5 years of progressive supervisory experience managing human service or case management teams.

Preferred Qualifications:

• Master’s degree in Social Work (MSW), Counseling, or related field.
• Active Michigan Social Work License (LLBSW, LBSW, LLMSW, or LMSW).
• Familiarity with the Transition to Independence (TIP) Model.
• Background in trauma-informed, strengths-based programming.

Additional Requirements:

• Valid, unrestricted driver’s license, reliable personal vehicle, active registration, and auto insurance (suitable for participant transport).
• Adherence to the NASW Code of Ethics and TNFC Code of Conduct.
• Flexibility to work occasional evenings/weekends and travel throughout Metro Detroit.
