Speech Language Pathologist (SLP) – Crest Home Health & HospiceCrestHome Health & Hospice is looking for a compassionate and skilled Speech Language Pathologist (SLP) to join our home health team.As a Home Health SLP, you’ll provide individualized speech and language therapy to patients in the comfort of their own homes. You’ll work closely with patients, families, physicians, and the home health team to help patients improve communication, maximize their independence, and reach their goals.What You’ll DoProvide skilled speech-language pathology services in patients’ homes according to physician orders and individualized plans of careComplete initial and ongoing speech and language evaluations and develop appropriate treatment goalsProvide therapy to improve or maximize patients’ communication, function, and quality of lifeDevelop, implement, and update individualized plans of careEducate patients and family members on treatment strategies and home programsMonitor patient progress and communicate changes in condition or treatment needs with physicians and the care teamComplete accurate and timely clinical documentation, progress notes, evaluations, and discharge summariesParticipate in interdisciplinary care planning and collaborate with other home health professionalsComplete required OASIS assessments and documentation as applicableUtilize appropriate ICD-10 coding and follow agency documentation requirementsParticipate in quality improvement and staff education activities as assignedWhat We’re Looking ForMaster’s or doctoral degree in Speech-Language Pathology from an accredited programCurrent Speech-Language Pathologist license in the state where services are providedMeets applicable state and federal requirements for practicing as an SLPTwo years of experience preferred, but we encourage qualified candidates to applyStrong communication, organization, and clinical documentation skillsAbility to work independently while collaborating effectively with patients, families, physicians, and the home health teamA compassionate, patient-centered approach to careWhy Home Health?Home health gives you the opportunity to build meaningful relationships with patients while seeing the impact of your work in their everyday environment.
