QUALITY ASSURANCE & IMPROVEMENT (QA&I) COORDINATOR

$35.00 per hour

Isoke Developmental Services is seeking a detailed-orientated and experienced Quality Assurance & Improvement (QA&I Coordinator to support the organization's commitment to high-quality, person-centered services for individuals with intellectual and developmental disabilities (IDD). This position plays a key role in monitoring regulatory compliance, evaluating service and documentation standards, identifying opportunities for improvement, and supporting ongoing organizational readiness for audits, licensing reviews and regulatory inspections. 

What You'll Do

• Administer and maintain ISOKE’s quality management and performance-improvement framework, including measurable objectives, data collection, analysis, reporting, and follow-up.
• Conduct scheduled and unannounced internal audits of residential, community-based, administrative, and personnel records to evaluate compliance with ISOKE policy, applicable waiver requirements, licensing standards, and 55 Pa. Code Chapters 6100 and 6400, as applicable.
• Review individual records for current plans, assessments, consents, service documentation, health and safety information, rights documentation, medication-related records, progress notes, and other required materials.
• Monitor employee-file compliance, including background checks, health screenings, credentials, orientation, annual training, competency documentation, driver requirements, and policy acknowledgments.
• Track audit findings, deficiencies, plans of correction, corrective and preventive actions, responsible parties, due dates, supporting evidence, and sustained compliance.
• Analyze incidents, complaints, grievances, medication errors, injuries, restrictive procedures, hospitalizations, staffing issues, and other quality indicators for patterns, root causes, and systemic risk.
• Collaborate with the Incident Management Representative and leadership to support timely, accurate incident reporting, investigation, finalization, corrective action, and trend analysis.
• Maintain current knowledge of ODP communications, regulatory updates, bulletins, guidance, waiver changes, licensing expectations, and quality initiatives; translate changes into operational action.
• Facilitate quality-management meetings and interdisciplinary reviews, document decisions, assignments, deadlines, and completion status.
• Provide coaching and technical assistance to managers and direct-support teams regarding documentation, individual rights, confidentiality, incident prevention, risk reduction, and regulatory compliance.
• Support development and revision of policies, procedures, forms, audit tools, training materials, and monitoring systems.
• Promote a culture of transparency, learning, accountability, person-centered practice, and continuous improvement without retaliation for good-faith reporting.
• Maintain strict confidentiality of protected health information, personnel information, investigations, quality records, and proprietary business information.
• Perform other duties assigned to support quality, compliance, and organizational operations.

Qualifications

• Bachelor’s degree in human services, social work, healthcare administration, quality management, business administration, psychology, nursing, or a related field; equivalent relevant experience may be considered.
• Two or more years of experience in quality assurance, compliance, incident management, auditing, program operations, or service coordination within intellectual/developmental disability, behavioral health, healthcare, or human-services settings.
• Working knowledge of person-centered services, individual rights, confidentiality, documentation standards, incident management, and continuous quality improvement.
• Ability to interpret regulations, policies, data, and documentation and convert findings into clear corrective actions.
• Proficiency with Microsoft Office or equivalent business applications, including Excel, Word, Outlook, and presentation software.
• Valid driver’s license and ability to travel to ISOKE service locations, meetings, and external reviews as required.
• Ability to satisfy all ISOKE, ODP, licensing, background-check, health-screening, and training requirements applicable to the position.
