Position Summary The Residential Program Supervisor (RPS) at QHCG provides programmatic and operational oversight across multiple assigned residential habilitation homes serving individuals with intellectual and developmental disabilities. This individual is the administrative and regulatory accountability layer above the House Manager level — responsible for multi-home compliance, staff development and performance management, payroll integrity, QAPI reporting, ISP outcome tracking, and direct liaison with QHCG administration, MCO Support Coordinators, and ODP monitors. This is not an entry-level role. The ideal candidate is a seasoned human services professional with a working command of Pennsylvania ODP regulations (Chapters 6100/6400), direct experience managing staff and programs across multiple IDD residential sites, and a demonstrated ability to run compliant, person-centered homes without daily administrative direction. The Residential Program Supervisor reports directly to the Administrator/Associate Administrator. Key Responsibilities Resident-Centered Care & Individual Programming Maintain thorough, current knowledge of every resident's ISP, care plan, behavioral support plan, diagnosis, history, and primary habilitation goals across all assigned homes Participate in the individual plan process, development, team reviews, and implementation; provide and supervise activities for individuals in accordance with their individual plans; and support integration of individuals in the community Review and approve monthly activity calendars submitted by House Managers for each assigned home; ensure alignment with resident ISP goals prior to posting and confirm a minimum of four community integration outings per month per home are planned and executed Review daily and weekly routine implementation across all assigned homes through scheduled and unannounced site visits; hold House Managers accountable for consistent execution of structured routines covering meals, hygiene, personal care, recreation, vocational skill building, and life skills Participate in or lead ISP annual review meetings; document and report measurable quarterly progress toward all active habilitation goals for all residents Analyze quarterly ISP progress data submitted by House Managers; identify residents not meeting participation goals and direct House Managers on required outcome-based activity modifications Uphold and document resident rights in compliance with ODP/OLTL standards across all assigned homes; address, document, and resolve grievances per QHCG policy Communicate with residents' families on a regular basis to provide updates on programming, health status, and goal progress Maintain proactive, ongoing communication with all assigned MCO Support Coordinators; serve as the primary home-level contact for Support Coordinator inquiries and ISP updates across all assigned homes Staff Supervision, Development & Performance Provide direct supervisory oversight to House Managers and all Direct Support Professionals (DSPs) and Home Health Aides (HHAs) assigned across all homes Monitor staff for effectiveness and professionalism; ensure Chapter 6400 regulations are followed and adhered to across all assigned homes Conduct periodic skip-level observations across all assigned homes; document findings and provide written feedback directly to the relevant House Manager for follow-up and corrective action Review and verify House Manager-submitted competency evaluation completion for all DSP staff across all homes; maintain the master training due-date matrix and elevate gaps or overdue certifications to administration Administer and document corrective counseling for attendance and performance issues using QHCG HR forms; elevate to administration when appropriate Lead formal staff meetings with written agendas, documented minutes, and follow-up action items Conduct mid-probation check-ins for all staff within their first 90 days; prepare annual performance reviews for DSPs reaching their one-year anniversary Mentor House Managers and Lead DSPs for supervisory growth and succession readiness Work closely with the Program Director and administration to schedule staff meetings, coordinate trainings, and problem-solve issues that arise across shifts and across all assigned homes Scheduling & Staffing Management Independently publish a two-week rolling schedule for all assigned homes on time, every pay period Develop and maintain a documented on-call rotation; communicate it clearly to all staff across all homes Implement and enforce a shift-fill protocol for call-outs with documented escalation steps Monitor and proactively flag overtime before it is incurred across all homes; adjust coverage to protect budgeted hours and staffing ratios Ensure 1:1 and 1:2 staffing ratios are maintained per each resident's active service authorization at all times Identify chronically absent staff across all homes and initiate documentation or counseling processes in line with QHCG HR policy Payroll & EVV Oversight Reconcile EVV clock-in/out data against scheduled shifts each pay period for all homes; resolve all discrepancies before submission Track PTO usage and balances for all staff across all homes; flag issues to HR promptly Maintain an ongoing payroll discrepancy log available for audit review at any time Understand and be able to clearly articulate the relationship between billable hours and payroll hours to an auditor or ODP monitor Regulatory Compliance & Documentation Ensure full compliance with Pennsylvania Title 55 Chapters 6100 and 6400 across all assigned homes at all times Ensure all daily Therap and EIM documentation is completed within 24 hours with no gaps across all homes; conduct monthly audits for timeliness, narrative quality, and accuracy Audit all Medication Administration Records (MARs) monthly across all homes for completeness and accuracy; zero missed signatures or documentation gaps are acceptable Monitor all residents' medical, physical, dental, vision, and psychiatric appointment compliance; coordinate the scheduling and documentation of all required appointments per Chapter 6400 timelines File all incident reports within 24 hours of any reportable event via HCSIS, applicable MCO portals, and internal QHCG forms; track all open corrective actions through to resolution Verify all residents across all homes have current physicals, dental, and vision appointments on file within required regulatory timeframes Confirm fire drill logs are current, completed monthly, documented, and signed by all staff at each home Maintain and reconcile petty cash and resident funds ledgers across all homes; submit balanced, documented audits quarterly Independently respond to ODP monitoring visits, OLTL reviews, and unannounced inspections without administrative prompting; maintain home readiness at all times across all assigned sites Physical Site Inspections & Safety Conduct monthly home inspections for each assigned site covering fire and life safety, interior environment, resident rooms, health and medical readiness, documentation compliance, and exterior/vehicle — using QHCG's standardized inspection checklist Complete quarterly inspections co-signed by both the Residential Program Supervisor and the Administrator; file all reports in the home record Identify and report all maintenance, safety, or environmental deficiencies immediately; track corrective action through resolution Verify fire extinguisher inspection dates, smoke detector function, egress paths, and posted emergency evacuation plans on a monthly basis at each assigned home Confirm vehicle registration, inspection sticker, and insurance are current at each home where applicable Quality Assurance & Reporting Present a quarterly QAPI data summary to administration covering incident trends, documentation compliance, staffing ratios, and resident activity and goal outcomes across all assigned homes Identify at least one process improvement opportunity per quarter and propose a written corrective solution to administration Track and report ISP goal progress data for all residents quarterly to the interdisciplinary team Submit all required monthly compliance reports: incident summary, staffing ratio logs, and medication administration records Review billing exceptions from EVV and work with the billing coordinator to resolve all outstanding items each billing cycle Required Qualifications High school diploma or GED required; Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Education, or a related field required Minimum 3 years of direct experience in a residential habilitation, community home, or IDD setting Minimum 2 years of demonstrated supervisory experience over direct care staff in a licensed residential program, including oversight of House Managers or equivalent site-level leads In-depth working knowledge of Pennsylvania ODP Chapters 6100 and 6400; ability to apply regulatory standards independently in day-to-day operations across multiple sites Experience with medication administration and MAR documentation in a residential setting Demonstrated ability to independently manage multi-home staff scheduling, payroll processes, and EVV compliance Familiarity with incident reporting platforms including HCSIS and EIM (Enterprise Incident Management) Valid Pennsylvania driver's license with a clean driving record Ability to pass Pennsylvania criminal background check, child abuse history clearance, and FBI fingerprinting as required under applicable regulations Current CPR/BLS certification (or willingness to obtain prior to start) Strong written and verbal communication skills; able to produce professional documentation, reports, corrective action plans, and meeting minutes independently Preferred Qualifications Prior experience as a Residential Program Supervisor, Program Lead, or equivalent title in a Chapter 6100 or 6400 licensed residential setting with multi-site oversight responsibility One or more years of direct work experience supporting individuals with intellectual disabilities or autism in a supervisory or programmatic capacity Proficiency with Therap EHR platform for resident documentation and individual records management Experience with UKG or a comparable workforce management platform for multi-home scheduling and payroll processing Knowledge of OLTL/Community HealthChoices waiver program requirements and MCO support coordination processes Experience with EVV platforms including HHAeXchange or Sandata Experience preparing for or independently responding to ODP licensing inspections or CARF accreditation reviews Familiarity with QAPI frameworks and quality improvement reporting in a residential habilitation context NADSP E-Badge certification or Frontline Supervisor training completion Bilingual candidates strongly encouraged to apply What Success Looks Like in This Role Within 30 days, this person knows every resident across all assigned homes by name, diagnosis, and ISP goals; has completed all required onboarding training; has submitted an initial site safety inspection for each assigned home, an open corrective action tracker, and a schedule gap report to administration; and has established working contact with all assigned MCO Support Coordinators. Within 60 days, this person independently publishes schedules for all assigned homes, owns the payroll and EVV reconciliation process every pay period without prompting, has reviewed and verified House Manager-submitted competency evaluation status for all DSP staff with at least 50% complete across all homes, and has completed at least one documented skip-level observation per home. Within 90 days, this person runs all assigned homes without daily oversight, has presented a first QAPI quarterly summary to administration, has verified full competency evaluation completion across all homes via House Manager submissions, and has passed the 90-day performance review with multi-site role readiness confirmed.
