Physical Therapist (PT) – Home Health Cleveland East Side

Platinum Home Health Services is seeking a compassionate, motivated Physical Therapist (PT) to join our growing home health team. The ideal candidate is committed to providing high-quality, patient-centered care while helping patients improve mobility, function, and overall quality of life in the comfort of their own homes.

- Perform comprehensive physical therapy evaluations and develop individualized plans of care
- Provide skilled physical therapy services in accordance with physician orders and agency policies
- Assess patient mobility, strength, balance, and functional limitations
- Implement therapeutic exercises, gait training, and pain-management techniques
- Educate patients and caregivers on treatment plans, safety, and home exercise programs
- Document evaluations, progress notes, and discharge summaries accurately and timely
- Communicate effectively with physicians, case managers, and interdisciplinary team members
- Follow all state, federal, and Medicare home health regulations
- Current Physical Therapist license in the State of Ohio
- Graduate of an accredited Physical Therapy program
- Home health experience preferred, but not required
- Strong clinical, communication, and organizational skills
- Ability to work independently and manage a caseload
- Valid driver’s license, reliable transportation, and auto insurance
