California Licensed Lead Case Manager

Providing comprehensive case management and support, the full-time hybrid California Licensed Lead Case Manager will assist individuals with persistent mental illness and/or substance use by developing care plans and connecting them to necessary services in West Los Angeles. Key responsibilities Serves as the primary point of contact for members and their support networks, coordinating care with a multidisciplinary team Assesses member needs and develops comprehensive Care Management Plans to address gaps in treatment and services Implements care plans, advocates for members, and connects them to social services and supports as needed Required qualifications High School Diploma or GED required; Bachelor's degree in social work, psychology, counseling, nursing, or related field preferred 2+ years of experience working with Medi-Cal members in care management or mental health services Current California driver's license and acceptable driving record Familiarity with trauma-informed care and harm reduction practices Ability to engage with diverse populations and work in various environments, including members' homes and clinical settings
