Home Health Aide

Job Summary:

NuChoice Health Partners is seeking compassionate and experienced Home Health Aides to join our team of dedicated caregivers. As a Home Health Aide, you will provide personalized care and support to clients in the comfort of their own homes. Our Care Providers are trained to assist with daily living activities, such as meal prep, companionship, light housekeeping, medication reminders, and personal care.

Responsibilities:

• Provide personal care services to clients, including bathing, dressing, and grooming

• Assist with daily living activities, including meal prep, light housekeeping, and companionship

• Administer medication reminders and assist with medication management

• Promote independence and dignity for clients while maintaining a safe and healthy environment

• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

• Maintain accurate records and documentation of client care services

Requirements:

• 0 years of experience in healthcare or a related field (training provided)

• Ability to lift 25 pounds and stand for extended periods

• Reliable transportation and a clean driving record (if applicable)

• High school diploma or equivalent required

• Additional Requirements: Meal prep, companionship, light housekeeping, medication reminders, personal care
