Primary Purpose

The LPN has the responsibility for the complete care of the residents residing at Beaumont. The LPN must coordinate the care between the resident, health care provider, family and nursing staff. LPN ensures that the necessary physical and psycho-social needs are met by maximizing the resident’s existing abilities, promoting independence and creating an atmosphere of autonomy.

 

Duties and Responsibilities

• Required to be cross-trained in all areas including Skilled Nursing, Personal Care and Wellness. Must be willing to work in all areas as needed.
• Promotes each resident’s optimal functioning in their activities of daily living.
• Respects and supports each resident’s choices regardless of personal beliefs.
• Able to manage difficult situations with residents, family and staff.
• Is respectful and supports the individual resident’s life style and works with those preferences throughout their stay.
• Ensures each resident’s privacy and dignity.
• Upholds each resident’s right to be free from abuse, neglect, exploitation and misappropriation of property and will report any suspected cases.
• Upholds all HIPAA regulations.
• Upholds Federal and State regulations and standards of clinical geriatric practice.
• Informs and educates residents and families on each resident’s plan of care.
• Works in conjunction with TR and promotes staff to initiate and engage residents in additional leisure pursuits.
• Promotes collaboration with all departments to ensure good communication which will enhance all resident’s quality of life.
• Must be able to evacuate residents in an emergency.
• Demonstrates a working knowledge of long-term care regulations and survey process as it relates to their position.
• Able to perform an accurate systematic assessment of all residents, interpret and communicate findings to the appropriate health care provider.
• Documents accurately and appropriately in accordance with State and Federal regulations.
• Demonstrates the ability to receive and transcribe verbal and written physician’s orders.
• Ensures CNA performs nursing care and documentation associated with the residents needs and plan of care.
• Communicates with CNA any change in condition or plan of care.
• Establishes & maintains effective communication between all staff in all departments.
• Takes an active role in the disciplinary process.
• Demonstrates the ability to set priorities and problem solve difficult situations independently but seek appropriate assistance when necessary.
• Admits and discharges residents in accordance with State and Federal regulations.
• Facilitates the thorough and seamless flow of information from shift to shift.
• Promotes customer service, quality of care, teambuilding, collaboration, accountability, and fiscal responsibility.
• Conducts nursing rounds on unit, report any abnormal findings and address concerns as they arise.
• Actively participates in the day-to-day operations on the unit as demonstrated by answering call bells, leading programs and transporting residents.
• Effectively practice and document infection control procedures.
• Will document according to CMS, State and Federal regulations.
• Administers medication and treatments according to State and Federal regulations.
• Demonstrates sound knowledge of current geriatric standards of practice.
• Must perform annual medication and clinical competencies in accordance with State and Federal regulations.
• Must maintain a nursing license and CPR certification in good standing with all the requirements set forth by the state of Pennsylvania.
• Demonstrates knowledge Beaumont Employee Handbook and disciplinary process.
• Responsible for completing mandatory in-service programs as established through the Human Resources Department.
• Responsible for attending staff meetings or getting the information presented during meetings.
• Responsible for attending care conference, updating plan of care, nursing care instructions sheet and notifying family/responsible party of any change in care or condition.
• May be certified for IV and Venipuncture.
• Oversees clinical operations (resident conditions, medications, treatments, completion of care).
• Coordinates, manages directs and delivers hands on nursing care.
• Managing nurse and CNA assignments.
• Able to work with interruptions.
• Demonstrates prioritization expertise and communicates this to staff.
• Is an educator and clinical resource to staff. Communicates topics to the DON for staff education to improve quality of care.
• Manages interpersonal conflicts to the point of resolution.
• A role model for all other employees. Leads by example.
• Communicates openly and effectively with DON.
• Upholds Beaumont nursing policy and procedures and plays an active role in the disciplinary process.
• Upholds the Beaumont Employee Handbook polices.

 

 

Qualifications/Skills

 

• Must possess a current, unencumbered, active license to practice as an LPN in this state.
• Must be able to read, write, speak and understand the English language.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess leadership and supervisory ability and willingness to work harmoniously with 
• professional and non-professional personnel.
• Must possess the ability to plan, organize, develop, implement and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
• Must be patient, tactful and have a cheerful disposition and enthusiasm as well as the willingness to handle difficult residents.
• Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

 

Physical Requirements

 

• Works throughout the nursing services area (i.e. drug rooms, nurse’s stations, resident rooms, etc).
• Sits, stands, bends, lifts and moves intermittently during working hours.
• Is subject to frequent interruptions.
• Is involved with residents, personnel, visitors, government agencies/personnel, etc under all conditions and circumstances.
• Is subject to hostile and emotionally upset residents, family members, personnel and visitors.
• Is subject to infectious waste, disease and other unpleasant conditions including TB and the AIDS and Hepatitis virus.
• Must be able to evacuate residents in an emergency.

 

Education

• Successful completion of accredited LPN Program.

Other Duties

Must comply with the general attendance policies.
