Key Responsibilities

Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs and physician direction
Collaborates with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services
Completes medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines
Manages individual, concurrent, and group treatment sessions effectively based on patient care needs and demonstrates flexibility in caseload management
Participates in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
Adheres to discipline-specific State Practice Act and all regulatory requirements; maintains

active professional licensure in good standing

and completes all state-mandated continuing education requirements
Required Qualifications

Active Physical Therapist license in good standing in the state of Ohio
Ability to complete comprehensive patient evaluations and develop clinically sound treatment plans and goals
Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills

Experience working within an interdisciplinary team environment in an inpatient or acute rehabilitation setting
Familiarity with functional quality indicator assessments and letters of medical necessity
Active involvement in professional development, continuing education, and professional organizations
