Job Title: Registered Nurse Profession: RN Specialty: Hospice Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours, 0800 - 1630 Experience: Minimum 2 years License: Active Illinois or Michigan nursing license Certifications: Current American Heart Association HealthCare Provider BLS Must-Have: - Provides own transportation and auto insurance. - Maintains a valid driver's license. Description: The Registered Nurse is responsible for a scope of care that encompasses all ages from infancy through geriatric. The RN demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Coordinates interdisciplinary care to patients, which may include nursing, therapy, social work, home care aide, home medical equipment, and home infusion services as needed. The RN Case Manager is responsible for decision-making regarding the home health care the patient receives during their episode of care. Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management, and episode management.
