The Case Manager provides comprehensive, integrated, recovery oriented services to all assigned members. Adheres to all required documentation standards. Ability to relate to individuals who have severe impairments in their ability to function in the community. Ability to respond quickly, effectively and with equanimity to member crises and life situations. Knowledge of Los Angeles County Department of Mental Health’s Rehab Option model of service provision and documentation requirements. Basic computer skills and literacy are required to use the agency electronic health record. Staff must be able to logon, type, use a mouse and negotiate through various screens or windows to complete daily chart documentation or supervision requirements. Ability to work as a team member and to work with other agencies, community resources and member’s families. Position Requirements: Must be able to pass California State Department of Justice criminal background investigation and fingerprinting. High School Diploma or Associates Degree in mental health or related field Bachelor’s Degree in Social Work, Psychology or related field preferred. Must have at least six years of experience working in the field of mental health/social services with a HS Diploma. Must have at least four years of experience with an AA degree in the field of mental health/social services. Must have at least two years of experience with a BA degree in the field of mental health/social services. Must have valid California Driver's license and the availability of a car with adequate insurance. Primary Duties: Provide targeted case management and mental health services to selected members, linking them with all appropriate community services and working with other service providers. Participate in outreach efforts to engage new clients in the MHSA program. Services shall be delivered in the community and in a manner that promotes client independence, resiliency, health and recovery of their mental illness. Involve members in all aspects of the services they receive as partners, providing them with choices and with opportunities for empowerment whenever possible. Ensure members are involved in establishing recovery oriented goals in the areas of living arrangements, social relationships, constructive use of leisure time, productive activity, and dental and medical health care. Provide mental health rehabilitation services, such as, counseling, money management, social skills training, and other independent living skills as needed by assigned members. Assist clients in developing skills associated with completion of weekly grocery shopping, household chores, and other independent living skills as required. Work as a team with agency psychiatrists, nursing staff and any other inter/intra agency staff to provide consistent services. Coordinate with the Money Management Specialist to assist client’s in obtaining or restoring social security &/or other entitlement program benefits as appropriate. Work as a team with the Money Management Specialist and client to support and develop money management plans and skill building activities. Responds to crisis, emergency and urgent care issues to clients in the program. Interventions should be made in an effort to promote client safety, recovery and resiliency and to avert hospitalization and /or incarceration. Maintain contact with member’s family and/or significant other support persons as appropriate. Maintain scheduled case management service meeting with all assigned members on caseload. Maintain a UOS log on a daily basis, documenting all services to be billed and attaching the progress notes for these services. Service codes and time billed must be consistent on the UOS log and progress note. Documentation is to be completed and submitted in a timely manner. In partnership with the client and their family (if desired) develop annual and six month service plan goals aimed toward recovery &/or increased resiliency based on member's established Coordination Plan cycle dates. Bill at least four to six hours of services daily as instructed by supervisor. Prepare for weekly clinical staff meetings having current information on the status of assigned members. Track members’ medication to ensure that they are medication compliant and that they have an adequate supply of medication at all times. On an ongoing basis, complete all Rehab Option, Outcomes study and other paperwork within prescribed time frames. Other duties as assigned. Environment/Working Conditions Routine duties involving interacting with a variety of individuals within the agency, clients, and the community. Daily functions which may include exposure to clients/family members from various ethnic, cultural and socio-economic backgrounds, with communicable diseases and some may be prone to violence. Strives to be flexible and adapt to change. Seeks opportunities to facilitate change and encourages others to do the same. Maintains a constructive and positive outlook. Travels to site locations and outside locations as needed or requested. Position requires driving, valid driver’s license, a motor vehicle clearance, and proof of insurance is required and must be maintained throughout employment.
