Impact Health is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing clinical team on a Per Diem (PRN) basis serving patients within skilled nursing care facilities in New Haven, CT!!

Compensation and Benefits:

• Earning potential: 1099 (Fee For Service)

Key Responsibilities:

• Assess, develop and communicate comprehensive treatment plans using evidence based best practices
• Documentation within 24-48 hours along with placing relevant orders for diagnostics and medications
• Monitor and manage acute and chronic conditions
• Compliance with facility, state and federal regulations/protocols.
• Support quality initiatives and process improvement plans.
• Collaborate with interdisciplinary team, nurse managers, physicians and specialists
• Educate and consul patients and families regarding course of treatment

Qualifications

Qualifications:

• Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
• Minimum 2 years of post-certification experience
• Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
• Proficient with EHRs and clinical technology
• Strong communicator with a collaborative mindset
