Home Health Registered Nurse (RN) | Denver, Colorado

As a Home Health Registered Nurse, youll deliver individualized, high-quality care directly to patients in their homes, ensuring continuity throughout their treatment journey. This role requires autonomy, organizational excellence, and a passion for patient-centered healthcare.
Schedule & Job Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 25-32 points per week (generally 3-5 home visits per day)
- Field-based position with daily travel within your assigned territory
- Electronic documentation in Home Care Home Base (HCHB)
- Comprehensive OASIS assessments required
- Mileage reimbursement for travel
Key Responsibilities:
- Complete and document OASIS assessments to develop individualized care plans
- Provide skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
- Monitor progress, promptly communicate patient status to care teams and physicians
- Educate patients and families about disease, treatment, and preventive measures
- Participate in collaborative care with physical therapists, social workers, and aides
- Maintain accurate records and adhere to home health compliance and regulatory standards
- Engage in ongoing education, case conferences, and staff meetings
Required Qualifications:
- Active RN license in good standing
- 1+ year of recent field-based Home Health RN experience
- Proficiency with HCHB and OASIS
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical, assessment, and time management skills
- Ability to excel independently in a mobile, patient-focused setting
