RN Team Nurse Full Time Direct Patient Care Zanesville, OH, US 5 days ago Requisition ID: 5418 What You Should Know About the RN Team Nurse Role: This is a full-time Hospice Team Nurse position. Schedule: 8:30am - 4:30pm with weekend and holiday rotation. The RN Team Nurse is a DRIVING position primarily based out of Zanesville, Ohio. The RN Team Nurse's Essential Duties Are: The RN Team Nurse carries the responsibility of flexibility in diverse Ohio’s Hospice RN roles as a float team member assisting Care Partners, Coming Home, Extended-Hours/On-Call and the Admission Departments and IPU as needed. Float team staff will collaborate with various IDT members both internal and external of the organization. Clinical services will be scheduled in a timely fashion according to the patient’s plan of care, observing patient’s continuity of care to achieve the desired outcome of comfortable and peaceful death for patients. When in the field, staff will provide excellent clinical care to all Ohio’s Hospice patients that meet the needs of our patients/families, Ohio’s Hospice standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients. Qualifications: Registered Nurse (RN) License in the State of Ohio without any board actions bachelor's degree preferred Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.
