Key Responsibilities\n

• Conduct thorough assessments of patients' social, emotional, and environmental challenges to develop personalized intervention strategies.

• Create and implement service plans that align with patients’ goals, including ongoing monitoring of their progress.

• Provide individual and family counseling, helping clients navigate complex emotions and circumstances during end-of-life care.

• Deliver crisis intervention services to patients experiencing acute distress, ensuring safety and effective resolution of immediate issues.

• Actively collaborate with external community agencies to coordinate services and advocate for patient access to necessary resources.

• Engage in community outreach initiatives to elevate awareness about hospice services and available psychosocial support.
\n
• Participate in team meetings, case conferences, and professional development sessions to contribute to a culture of learning and improvement.

• Document all patient interactions and maintain accurate records in compliance with healthcare regulations and organizational policies.

Patient Population / Client Focus\n

Your work will primarily focus on serving patients in hospice care, who are facing significant physical and emotional challenges associated with terminal illnesses. You will support not only the patients but also their families, helping them process grief and fostering a supportive environment during one of the most challenging times in their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is essential for the position.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n
