Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes. Provide education to patients and their families regarding therapeutic objectives, discharge strategies, and home exercise routines. Accurately document evaluations, therapies, and patient progress as per facility and regulatory requirements. Engage actively in care team meetings and interdisciplinary conferences to ensure comprehensive patient care.

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution. State licensure as a Physical Therapist. Preferred CPR certification. Any specialized certifications such as neurological or geriatric-focused are advantageous but not mandatory.

Previous experience in a similar role is highly preferred. Adeptness in treating complex medical and mobility-related issues. Excellent communication and teamwork abilities. Comfortable utilizing electronic documentation systems for clinical charting.
