Job Summary

Outpatient Physical Therapist in a healthcare setting. Job Details

• Position Description: Evaluates all assigned patients and develops, implements, and modifies treatment plans aligned with patient needs and physician instructions. Responsible for delegation and direction of therapy services for assigned patients. Ensures the design of functional programs that facilitate patient reintegration into community activities. Serves as an educator for patients, families, students, and team members. Performs professional duties related to patient care, therapy service development, and program design. Demonstrates behaviors aligned with organizational Mission Statement and Core Values, ensuring compliance with the Code of Conduct, confidentiality, and professional standards.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 2 Years of Outpatient Clinical Experience; CPR Certification; Location must be within a feasible commuting distance; 0 Years of minimum experience required.
• Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
• Additional Information: Completes comprehensive patient evaluations, develops clinical treatment plans, ensures accurate and timely documentation, and provides care under signed physician orders. Participates in student programs, fosters professional growth, and maintains licensure and certification compliance.
