Details Acute care setting in a Level 2 trauma hospital Care Coordination/Case Management unit covering all areas of the hospital, excluding outpatient and home care Handles 15-20 patients/consults per day across all payer types, including uninsured and homeless populations Uses Cerner EMR and Interqual criteria for utilization review 8-hour day shifts, scheduled between 8am-5pm depending on department Every other weekend required, with at least one full weekend per month Holiday coverage not required but welcomed if willing First to float among staff Business casual attire required Hospitalist/Intensivist and Respiratory Therapy available 24/7; pharmacy not available 24/7 Requirements Active CA RN license required BSN required BLS certification required Minimum 2 years of RN experience required Prior recent CA Dignity Health experience required for consideration Familiarity with Interqual criteria required Experience with both discharge planning and utilization review required Strong collaboration, teamwork, and communication skills with ability to reprioritize throughout the day Copy of driver's license required at time of consideration, reflecting legal name with no middle initial or middle name Two supervisor references required, one from within the last 12 months worked and one from within the last 3 years worked Additional Information Responsibilities include completing initial assessments, utilization review (IQ), discharge planning, and charting in Cerner Both utilization management and discharge planning duties are required Training provided for Interqual (IQ) criteria 5 days of care coordination new employee orientation, with unit orientation timeframe varying case by case Candidates may not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit-affiliated facility, including CHI or Dignity, within the past year Candidates employed PRN through outside staffing firms will be considered American Traveler Job ID P-748216.
