Travel Physical Therapist (PT)

We are seeking a dedicated Physical Therapist for a home health role in Jeffersonville, IN. Key Responsibilities Include Providing one-on-one, patient-centered care in patients’ homes, focusing on improving mobility, strength, balance, and functional independence. Conducting evaluations, developing individualized plans of care, and implementing evidence-based interventions. Performing home safety assessments, recommending environmental modifications, and educating patients on fall prevention and home exercise programs. Collaborating with an interdisciplinary team, ensuring clear communication and thorough documentation. Traveling between patient homes, managing a structured caseload, and maintaining work-life balance. Qualifications Include Minimum of 1 year of home health physical therapy experience. Valid PT license, with PT (Compact accepted). Required: SSN, DOB, and at least one professional reference. Skills And Attributes Strong communication, documentation, and clinical skills. Ability to work independently, demonstrate clinical autonomy, and build rapport with patients and families.
