Path to Wellness, Inc. is seeking a dedicated Full-Time Licensed Clinical Social Worker (LCSW, LMHC, LMFT) to join our compassionate team. As a Full-Time Licensed Clinical Social Worker, you will participate in virtual team meetings, client sessions, and interdisciplinary care discussions. Key Responsibilities\n- \n
- Conduct ongoing assessments of patients’ needs, ensuring that care plans are tailored to meet evolving circumstances, always in collaboration with patients and their caregivers. \n
- Formulate a comprehensive treatment plan after detailed initial assessments, with specific goals and measurable outcomes established in partnership with physicians. \n
- Deliver therapeutic interventions, including crisis counseling, for individuals, groups, and families, demonstrating sensitivity and responsiveness to each unique situation. \n
- Facilitate individual psychotherapy sessions and lead group therapy discussions, applying evidence-based practices to support patient outcomes. \n
- Maintain strict adherence to organizational policies, mental health standards, and regulatory compliance within the scope of professional practice. \n
- Evaluate individual and family systems to assess functional capabilities in daily life, work, and relationships, identifying areas that require additional support and resources. \n
- Regularly review and adjust treatment goals based on patient progress, ensuring to document all evaluations and modifications promptly. \n
- Support educational outreach and resource identification for families, empowering them with knowledge about mental health care processes. \n
You will work with a diverse population of patients who are facing terminal illnesses and end-of-life issues, alongside their families. This role requires a strong ability to provide empathetic support and guidance, helping patients navigate their emotional and psychosocial challenges during a critical time in their lives. You will facilitate conversations around grief, acceptance, and quality of life, contributing to a supportive environment that honors individual experiences and choices.
Qualifications\n- \n
- A valid Social Work License (LMHC, LMFT, LICSW) as mandated by Washington state regulations. \n
- A Master’s Degree in Social Work from an accredited institution recognized by the Council on Social Work Education. \n
- Exceptional communication and interpersonal skills, with the ability to create rapport with clients from varied backgrounds. \n
- Proficient in conducting assessments, developing care plans, and implementing therapeutic interventions consistent with best practices. \n
- Familiarity with the specific mental health and substance abuse standards pertinent to hospice care. \n

- Prior experience in hospice, palliative, or end-of-life care is highly advantageous. \n
- Experience in working with patients facing complex mental health issues and various socioeconomic backgrounds. \n
- Demonstrated ability to collaborate within a multidisciplinary team environment. \n
- Knowledge of community resources to support families and caregivers during bereavement. \n
The salary for this position ranges from USD 80,000 to 122,000 annually, commensurate with experience and education.
