Disability Services Case Manager — Community Access

State of Iowa

The Iowa Department of Health and Human Services, Division of Aging and Disability Services, seeks a Disability Services Case Manager based in Cherokee, IA to assist individuals with disabilities in accessing resources. The role includes developing care plans, coordinating with agencies, and navigating the service system to promote independence.
