Responsibilities
- Provide bedside nursing care for assigned residents under supervision
- Administer medications and treatments per physician orders and facility policy; document accurately in the electronic medical record
- Monitor residents’ conditions, identify changes, and report to the nursing and interdisciplinary teams
- Assist with CNA duties as needed and participate in developing and following individualized care plans
- Maintain safety, infection control, and resident privacy; respond promptly to call lights
- Collaborate with the care team to ensure continuity of care and proper documentation

Requirements
- Active New York LPN license or proof of application with applied date
- AHA BLS certification
- Minimum of 1 year of LTC experience
- Ability to cover CNA duties as needed
- Availability to work evenings; may be required to work weekends and 2 out of 4 holidays (Thanksgiving, Christmas, Christmas Eve, and New Year’s Day)
- Reliable transportation required; public transportation not available in the area
