Advance Practice Provider - Primary Care

We are seeking a dedicated Advance Practice Provider (Nurse Practitioner / Physician Assistant) to join a collaborative outpatient primary care team.

Key Responsibilities:

\n

\n
• Provide primary care for routine and acute conditions.
\n
• Conduct annual wellness visits and health risk assessments.
\n
• Coordinate patient care with specialists and healthcare teams.
\n
• Educate patients on diagnoses, treatments, and care plans.
\n
• Support population health initiatives and patient outreach.
\n

Required Qualifications:

\n

\n
• MSN, DNP, NP, PA-C Certificate qualification.
\n
• National certification in an eligible PA-C /NP specialty or Physician Assistant certification.
\n
• Active, non-probationary state NP license (for NPs).
\n
• Active DEA license.
\n
• Authorization to work in the United States.
\n
• Fluency in Spanish is required.
\n

Preferred: Experience in primary care, internal medicine, geriatrics, or family medicine, with knowledge of ICD-10 coding, HCC documentation, and health risk assessments.
