Description

• Conduct assessments for patients in both inpatient and outpatient environments, creating personalized treatment plans tailored to individual needs
• Implement therapeutic interventions that enhance mobility, strength, balance, and overall functionality
• Engage in collaborative efforts with a multidisciplinary team including nurses, physicians, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and their families on rehabilitation goals, discharge protocols, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and patient progress in accordance with facility policies and regulatory standards
• Actively participate in collaboration rounds and interdisciplinary meetings to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Rehabilitation Specialist
• CPR certification is preferred
• Additional specialized certifications (e.g., in neurology or geriatrics) are welcomed but not required
