Hospice Case Manager

RN Case ManagerA well-established hospice organization is seeking a full-time RN Case Manager to provide compassionate, patient-centered care in a community-based setting. This role is ideal for a nurse who values meaningful work, interdisciplinary collaboration, and balanced caseloads.The RN Case Manager is responsible for coordinating and delivering hospice care services to assigned patients while supporting families through the end-of-life journey. This role works closely with an interdisciplinary team to ensure quality, compliance, and continuity of care.Key ResponsibilitiesManage a caseload of approximately 10–14 patientsConduct patient assessments and develop individualized care plansCoordinate care with physicians, social workers, aides, and other team membersProvide education, emotional support, and guidance to patients and familiesComplete timely and accurate clinical documentationParticipate in interdisciplinary team meetings as neededQualificationsActive Registered Nurse (RN) license in the state of employmentMinimum 1–2 years of nursing experience (hospice or home health preferred)Strong clinical judgment and organizational skillsEffective written and verbal communication abilitiesAbility to work independently while collaborating within a team environmentValid driver's license and reliable transportationPreferred CertificationsHospice and Palliative Nursing certification (preferred, not required)Current CPR certificationSkills & CompetenciesUnderstanding and acceptance of hospice philosophyStrong interpersonal and relationship-building skillsCompassionate, patient-focused approach to careAbility to manage time and priorities effectivelyCompensation & BenefitsCompetitive payPaid time off and health benefitsRetirement plan with employer contributionSupportive and mission-driven work environment
