Position Summary: The Patient Care Navigator - MOUD is responsible for collaborating with medical and behavioral health staff to support HealthPoint’s MOUD program. The Patient Care Navigator - MOUD will work with clinics, and the addiction medicine care team, to optimize our ability to treat patients with OUD with Suboxone, Vivitrol, or other treatment modalities focusing on patient engagement and harm reduction practices. Compensation is dependent on skills and experience. Your contribution to the team includes: Ability to problem solve using critical thinking skills, judgment abilities, visionary capabilities, and creativity as it relates to supporting HealthPoint’s MOUD program. Support and deliver MOUD program with an anti-oppression, harm reduction lens. Access the Washington State Prescription Drug Monitoring Program database as needed to support staff and patients. Collect pertinent data to support HealthPoint’s MOUD program and patient. Provide patients with resources to other community services including addiction, mental health and primary care as needed. Assess, document, and implement actions required to meet patients and their families social, emotional and support needs. Work with various families and groups as is required to provide supportive services. Work with patients of all ages, abilities, and life circumstances in varying and unpredictable situations. Demonstrate leadership ability and has experience and willingness to lead change. Maintain good attendance, is punctual and works full scheduled shift is a condition of employment. Demonstrate respectful, professional and appropriate behavior that supports a team-oriented work environment. Demonstrate a commitment to the mission, core values and goals of HealthPoint and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services. Provide outreach and harm reduction services to patients with opioid use disorder. Engages patients’ significant support systems. Activities include assessment of patient and families social, emotional and support needs, intake/admission, evaluation, triage, teaching, and monitoring of patients in our OUD program. Work with medical and BH providers to transition unstable OUD patients to higher levels of care within the community. Track and analyzes data related to patient population and develops a plan of action and monitors progress over time and initiates changes as needed. Track and analyze data related to the OUD program at HealthPoint. Assist in the management of OUD patients following established protocols and systems for disease management in collaboration with providers. Communicate with a multidisciplinary team (physicians, advanced practice clinicians, nurses, behavioral health consultants, etc.) as needed to assist with disease management. Utilize patient-centered communications strategies, e.g., motivational interviewing. Involve the patient in developing a plan of care, goals or other specific measures pertinent to their health condition. Assess patient activation and readiness for change and collaborates with patient to develop self-management goals. Actively participate in all leadership team activities. Act as a liaison between clinics, administration, and community resources, as needed. Perform other duties as assigned by supervisor. Must have’s you’ll need to be successful: Equivalent education and experience and expertise relevant to a four-year degree in social work, health education, health care management or a similar field of study. Ability to read and interpret technical and other complex documents. Ability to write routine correspondence such as letters and memos. Must have strong customer service skills with the ability to present information in one-on-one and small group situations to internal and/or external clients. Understand interdisciplinary team concepts. Ability to apply principals of logical or scientific thinking to a wide range of intellectual and practical problems. Work situations are vaguely defined, often unprecedented, and unique in character. Problems are very complex, abstract, conceptual, and long-term in nature. There is a continual requirement for strategic, innovative thought and synthesis of information as it pertains to work situations. Work situations require consideration and interpretation of circumstances or information to choose the most effective response. Solutions may be technical yet relatively straightforward and well-defined once problems are understood. Possesses basic operating knowledge of computers. Beginning level of Word, Excel and Outlook required. Complete the Peer Support Program training through the HCA within six (6) months of hire. Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. If you have received the Johnson & Johnson vaccine, we ask that you provide documentation demonstrating proof of an alternate COVID vaccine or vaccine series. All new employees are also required to show proof of immunizations and/or immunity to MMR (measles, mumps, rubella), Varicella, annual Influenza and TB QuantiFERON Gold Titer . Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immune titer results prior to starting or no later than their fifth (5) business day of employment. Where to gather your records: If you are providing immunizations from an electronic health record, please ensure that you obtain a copy of your full records rather than a screenshot. Each page of your records should include your first and last name, date of birth, and the name of the health system from which the records are pulled. If records do not show any data, please seek guidance from your provider for further assistance. If you are unable to provide proof as noted above, you can choose to have a lab titer drawn to check immunity or to be re-vaccinated. If you receive vaccination(s) or lab titers, you may obtain them through HealthPoint at no cost to you.
