Accommodate, Restore & Collaborate, LLC is seeking a PRN Physical Therapist Assistant (PTA) for Meadowbrook Nursing & Rehabilitation in LaGrange, IL. The job duties include, but are not limited to, the following:
- Providing treatment based off evaluations/plans of care created by the supervising therapist.
- Appropriately and effectively completing documentation.
- Screening residents and individuals who may benefit from therapy.
- Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.
- Collaborating and updating the supervising therapist regarding the progress of the patients.
- Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.
- Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
- Supporting and participating in department operations/meetings.
Requirements are as follows:
- Licensed in the state of Illinois as a Physical Therapist Assistant.
- Ability to communicate well with patients, families, supervisors, and colleagues.
- Comfortable with computers.
- Preferred experience working in a SNF, but we are open to other practice area experience.
