The LUK Assessment Clinician works as part of a team to provide triage, initial and comprehensive assessments for individuals across the life span who are referred to the Certified Community Behavioral Health Clinic (CCBHC) for services. The Clinician ensures appropriate screenings and assessments are completed within a client driven, trauma informed framework with the intent of identifying services that support short- and long-term client goals. This position collaboratively develops and regularly reviews treatment plans with consumers, families, and collaterals as indicated. The Clinician will provide crisis management and short-term intervention as appropriate, working closely with the individual/family, internal and external team members. The Clinician will be expected to participate in training for EBPs as needed for this position, and ensure fidelity to the identified models. This is a full-time position with some non-traditional hours expected.

Purpose:

LUK’s CCBHC Assessment Clinician will be part of an innovative team seeking to ensure equitable, quality, person centered and strength-based service provision to consumers with mental health and/or substance use needs.

Responsibilities Include:

• Triage referrals that come in to LUK’s CCBHC
• Participate in the intake/assessment process for CCBHC consumers, meeting all LUK standards and contractual expectations
• Oversee the development and implementation of the treatment plan and progress reviews
• Provide trauma-informed clinical services (individual, family, crisis)
• Complete EBP training(s) and maintain fidelity to identified model(s)
• Maintain timely, accurate, strength-based, and trauma-informed documentation in compliance with agency and funding/regulatory entities
• Collaborate with internal and external members of a multidisciplinary team to support consumers and families in achieving their goals
• Participate in youth- and family-centered treatment team meetings
• Maintain timely and effective communication with consumers, families, and collaterals as indicated in the treatment plan and as needed
• Provide crisis management and intervention as appropriate

Job Location:

The Assessment Clinician will work at LUK’s Main Office in Fitchburg.

Salary Range and Benefits:
Pay Range: 8D. Salary Range: $62,000 – $72,500. Salary ranges within each pay grade are based on qualifications, skills, and experience.

Knowledge and Experience Required:

• Master’s Degree in a human services field
• Independent licensure preferred
• Experience and credentials in the delivery of services to individuals with SED, SMI, SUDs/CODs
• Experience implementing evidence-based practices
• Experience working with system-involved families
• Bilingual/bicultural candidates encouraged to apply
