Responsibilities Develop and maintain relationships with local hospitals, healthcare providers, and other stakeholders to ensure effective communication and collaboration Coordinate patient admissions, discharges, and transfers to ensure a smooth transition between care settings Participate in discharge planning and care conferences to ensure that patients receive comprehensive care Collaborate with interdisciplinary teams to identify and address patient needs, improving overall care outcomes Provide education and support to patients, families, and caregivers on discharge plans and post-acute care options Maintain accurate records and reports, ensuring compliance with regulatory requirements and facility policies Requirements High school diploma or equivalent required; degree in a related field (e.g., healthcare administration, social work) preferred 2+ years of experience in a hospital or healthcare setting; experience as a liaison or care coordinator a plus Excellent communication, interpersonal, and problem-solving skills Ability to work independently and collaboratively as part of a team Strong organizational and time management skills, with attention to detail and follow-through Why Join Grandview Nursing & Rehabilitation?
