In-Home Physician

Signify Health is seeking an In-Home Physician to perform comprehensive health evaluations for seniors and high-risk patients in their homes. You will conduct medical histories, physical exams, and medication reviews; identify care gaps; and document findings in EMR systems to support value-based care. Partner with an interdisciplinary team to coordinate follow-up care and referrals.
