We're seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services.The Licensed Practical Nurse works within the health care team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, the reduction of disability, improving the clients self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention, and provides health information/education to patients. Supports providers when staffing indicates the need.
