Clinicians in Action professional development program. Responsibilities Provide outpatient rehabilitation services in an in‑home setting for older adults. Schedule and manage patient care and billing documentation. Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel. Attend personnel and patient care meetings. Make referrals to other disciplines and agencies. Consult with rehab team members, including community physicians, and select therapeutic regimens. Adapt program and plan of care according to individual patient needs. Promote a safe environment and eliminate hazards. Order and maintain supplies and equipment. Ensure patients and families receive high‑quality service. Screen, examine, evaluate patients, establish diagnosis, and plan care. Implement therapeutic techniques in areas such as cognitive skills, muscle strength, coordination, endurance, mobility, perceptual abilities, sensory awareness, sitting and standing tolerance, balance, ADLs, joint protection, work simplification, orientation, physical agent modalities, low vision, fall risk management, medication management, health and wellness, community integration. Design, fabricate, fit, or apply adaptive devices and train patients. Select appropriate activities based on physical and mental capacity. Instruct patients in functional skills and evaluate progress. Perform routine reexaminations to modify plan of treatment. Perform other related duties as required. Qualifications Bachelor's degree in occupational therapy curriculum accredited by ACOTE. Initial registration from the National Board for Certification in Occupational Therapy. Valid and unencumbered OT license required in the state of practice. Valid driver’s license or reliable independent transportation to patients’ residences.
