Contract | 13-Week Assignment | Full-Time | Onsite | Skilled Nursing Facility (SNF)

Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore function and improve mobility
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
• Educate patients and their families on therapeutic exercises and home care strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of Missouri or ability to obtain prior to start
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing facility (SNF) or similar post-acute care setting
• Strong clinical documentation and communication skills

Preferred Experience / Skills

• Prior experience with geriatric or post-acute patient populations
• Familiarity with MDS and Medicare documentation requirements in a SNF environment
