Health Home Plus Care Manager

$3,000 per month

Title & Locations Health Home Plus Care Manager – Positions available in Middletown, Newburgh and Port Jervis. Reports to Care Coordination Program Supervisor. Job Objectives Provide care management to individuals enrolled in the Health Home Plus Care Coordination Program, working primarily with individuals with mental health issues and/or chronic medical conditions, while actively promoting an environment consistent with MHA’s Cultural Equity Policy and WELCOME Orange. Responsibilities Attend bi‑weekly Care Coordination team meetings. Attend trainings and meetings relevant to Health Homes and Health Home Plus. Deliver care coordination services as outlined under the Health Home Plus Model for Care Coordination. Complete New York State Community Mental Health (CMH) assessments and reassessments. Submit monthly billing for caseload. Complete monthly scorecard data collection. Maintain all client records electronically through FCM and AWARDS Electronic Record Keeping. Maintain all client files according to program policies and procedures. Establish and maintain linkage with appropriate community agencies and service providers. Represent MHA at meetings as necessary. Other Duties Answer MHA’s main office phone as needed. Participate in MHA’s committees as needed. Requirements Bachelor’s Degree in child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR a bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR a Credentialed Alcoholism and Substance Abuse Counselor (CASAC). Two years of experience (a Master’s degree in a related field may substitute for one year’s experience) providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse, OR linking these persons to a range of community services. Bilingual Spanish/English. Effective written and oral communication skills. Experience with Health Homes highly preferred. Knowledge of Medicaid procedures highly preferred. Experience with Electronic Health Record Program(s) required. Experience working with Excel and Microsoft Word preferred. Ability to work independently with minimal direct supervision. Must be organized, self‑motivated and able to coordinate multiple tasks simultaneously. Ability to exercise sound judgment under crisis situations and to abide by confidentiality regulations. Valid driver’s license with acceptable driving record. Terms of Employment / Benefits Non‑Exempt position. 40 hours per week. Competitive compensation rates. 1 hour paid lunch for full‑time employees. 4 wellness days annually for full‑time employees. 14 paid holidays, including 1 floating holiday. 403(b) plan with discretionary matching.
