divh2Bilingual LVN (Spanish) Opportunity/h2pWe are looking for a bilingual Licensed Vocational Nurse (LVN) for one of our managed care clients here in the South Bay. They are looking for the following:/ppUnder Pharmacist supervision, enhance seniors and their caregivers abilities to manage their medications more safely in the community to prevent unnecessary medication-related problems and negative health outcomes. You will:/pulliSupport the Pharmacist during medication management services by providing home-based and telephonic coaching on specific medication safety issues./liliFacilitate communication between the clients, their caregivers and their providers/service providers to address gaps in safe medication management./liliParticipate in departmental QA process and assist with program documentation./liliUnder Pharmacist supervision, maintain clients health by helping the client and caregiver understand and use medication and other health-related community services and reinforcing use of self-management and medication-related organizational tools. Under Pharmacist supervision support clients well-being by providing ongoing face-to-face or telephonic visits with clients to identify barriers to safe medication use, and supporting the plan of care developed. Contribute to the management and development of an individualized care plan for the client by conducting home visits, reporting and documenting home-based observations and providing approved coaching on identified medication safety issues, as directed by the Pharmacist. Under Pharmacist supervision, assist clients and caregivers in taking an active role in safe medication management by coaching, education, navigation and referrals to pharmacy and community medication resources. Identify pertinent information to communicate to care team by contacting client as needed./liliExhibit strong interpersonal, critical thinking and analytical skills through positive communication with clients, caregivers, care team and community resource agencies/organizations, including pharmacies. Demonstrate excellent organizational, decision-making and multi-tasking skills as demonstrated by problem solving and successful outcomes./liliComplete timely and accurate documentation in multiple computer systems to record assessment, observations and corresponding documentation, including care plans, and progress notes./liliEnhance skills and knowledge by participating in educational offerings and team case conferences and training per department guidelines./li/ulpYour qualifications:/pulliG.E.D. or High School Diploma/liliLicensed Vocational Nurse required with current and active California license in good standing/lili2+ years of in community services, preferably a healthcare or home health environment./lili2+ years working with older individuals and their families./liliWorking knowledge of medical and medication terminology and abbreviations/liliBroad understanding of local community medication-related resources for seniors and caregivers/liliOutstanding verbal and written communication skills/liliAbility to work in a fast-paced, every changing work environment/liliProficient in MS Office required (e.g. MS Word, MS Excel)/liliStrong analytical and critical thinking skills./li/ulpYour preferred qualifications:/pulliCommunity-based and home-based experience./liliProficiency in a second language./li/ulpWhats in it for you?
