Job Title Home Health Physical Therapist Location Davison, MI, USA Employment Details Employee Type: Full-time (40 hours per week) About the Role Residential Home Health and Hospice is a nationally recognized leader in home health and hospice. We are seeking an autonomous and clinically skilled Physical Therapist to deliver high-quality care to patients in their homes across Davison and surrounding areas. The position offers a total bonus potential of $20,000, including a first‑year success bonus and a sign‑on bonus. What You’ll Do Deliver skilled, patient‑centered care in a home health setting, collaborating with an interdisciplinary team. Perform comprehensive patient evaluations and functional mobility assessments. Develop individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies. Think critically, solve problems in real time, and maintain thorough, compliant EMR documentation. Work independently while traveling to patient homes within an assigned territory. One year of experience as a Physical Therapist. Current driver’s license and ability to spend approximately 20% of the day traveling to patient locations.
