Develop, implement, and adjust individualized treatment plans addressing mobility, gait, strength, balance, endurance, and functional outcomes. Provide direct patient care, supervise therapy sessions, and modify interventions based on progress and safety considerations. Document evaluation results, plan of care, progress notes, outcomes measures, and discharge summaries in a timely, accurate manner. Collaborate with nursing staff, therapists assistants, and interdisciplinary team members to coordinate care and optimize residents’ outcomes. Ensure patient and staff safety, adhere to infection prevention standards, and utilize equipment and modalities appropriately.
