Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community. As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants. The PACE Clinic Manager supports the mission and vision of Neighborhood Healthcare by overseeing clinic operations and cultivating an environment that delivers high-quality, patient-centered care within a dynamic, interdisciplinary team setting. This role is responsible for ensuring efficient clinical workflows, achieving quality and productivity goals, and supporting staff development through consistent leadership and alignment with organizational values. This position is fully onsite. Schedule: Monday-Friday, 8am-5pm. Responsibilities Oversees the day-to-day clinic operations, including staffing, scheduling, clinic staff supervision, and quality outcomes. Monitors staffing patterns, caseload distribution, and service coverage to ensure operational efficiency and appropriate workload balance Supports workforce planning, onboarding, and role development to meet evolving program needs Maintains competency in Interdisciplinary Team (IDT) processes and facilitates IDT meetings as directed, ensuring effective coordination of care and operational alignment Ensures standardization of clinic operations within scope; identifies, reports, and addresses variances with rationale to maintain alignment with program expectations Collaborate with department leadership to create, review, and revise department policies/procedures, as needed Creates and establishes a reoccurring training and orientation program for new hires and existing staff, including competency assessments to measure their ability to provide quality care to participants Ensures timely and accurate care to participants by managing clinic patient flow, overseeing appointment bookings, triaging, and gatekeeping with primary care and staff Manages and/or implements pharmacy and physician orders, including signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel Performs or supervises general skilled nursing care to participants, including administration of prescribed medications, treatments, and assistance with examinations according to nursing standards Assists nursing staff in providing direct care, including wound care, IV therapy, blood sugar checks, IM injections, vital sign checks, and/or follow-up on episodic problems identified by providers, such as congestive heart failure, chest pain, dehydration, edema, urinary tract infection, or other health problems Consults with and educates participants, family members, and/or caregivers regarding the disease process, self-care techniques, prevention strategies, and medical interventions Oversees and directs the coordination of all aspects of hospital, SNF, or ALF admissions Identifies emergency situations and takes appropriate action, including coordinating direct hospital admissions from clinic to hospital, as needed Maintains integrity of medical/operations records and complies with all data collections and auditing activities Maintains and monitors inventory systems for clinical supplies and clinical equipment Ensures compliance with healthcare standards and requirements established by various licensing, regulatory, and accreditation bodies Participate in committees that support program operations and quality improvement Ensure clinical supervision structures are clearly defined and maintained within the clinical team, with appropriate delegation and oversight in accordance with scope of practice and regulatory requirements Leads, coaches, and develops clinic staff, including hiring, onboarding, performance management, and disciplinary actions in coordination with Human Resources Provides directions to clinical staff and supervisors to ensure alignment with established workflows, performance expectations, and organizational priorities Conducts regular department meetings to promote communication, accountability, and alignment Evaluates staff performance and supports ongoing development through coaching, feedback, and competency-based assessments Ensures appropriate staffing coverage and operational support across clinic functions Promotes adherence to organizational mission, values, and service standards Partners with the Center Director and Quality and Compliance to review, investigate, and resolve participant grievances, appeals, and service concerns, ensuring timely resolution and appropriate documentation Leads operational planning and readiness for new center openings, Alternative Care Site (ACS) expansion, and program growth initiatives within assigned scope Provides coverage for staffing gaps, including LOA, PTO, and high-volume periods, by stepping in to support operations as needed Maintains effective communication and professional relationships with staff, participants, families, and external stakeholders Contributes to infection control and quality improvement activities Performs other duties as assigned Qualifications Education & Experience Bachelor’s degree in nursing or equivalent combination of education and experience is required Valid CA Registered Nurse license required and maintained as a condition of employment Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment AHA-approved courses include an in-person, hands‑on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques Online-only BLS courses without a live skills check do not meet this requirement. In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually Three years of healthcare experience as nurse with a background in geriatrics or long-term care is required Two years of supervisory experience within the hospital, long‑term care, or other related healthcare facility environment is required Additional Qualifications (Knowledge, Skills and Abilities) Excellent verbal and written communication skills, including superior composition, typing and proofreading skills Ability to interpret a variety of instructions in written, oral, diagram, or schedule form Knowledgeable about and experience with implementing new programs or start‑up environments Ability to successfully manage multiple tasks simultaneously Excellent planning and organizational ability Ability to work as part of a team as well as independently Ability to work with highly confidential information in a professional and ethical manner Physical Requirements Ability to lift/carry 10 lbs/weight Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. Two plus weeks of vacation, Nine Holidays including two Floating Holidays of your choosing, Sick/Personal time, Volunteer Time Off (VTO), 403b Retirement plan (similar to a 401k), optional Health and Wellness events, and much more! Pay range: $124,100 - $191,000 annually, depending on experience. Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs.
