This position requires weekday travel to assigned areas. Schedule: Monday-Thursday, 4-10's. Required: Prior Admissions experience in a Hospice or home health setting. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.Performs on-call responsibilities and on-call services to patients/families as assigned.Performs other duties as assigned.QualificationsRequired:Prior Admissions experience in a Hospice or home health settingCurrent RN license in the state(s) where you will workMinimum 1 year of clinical RN experience in an acute healthcare settingCurrent CPR certificationPreferred:Strong communication and interpersonal skillsAbility to work independently and manage multiple prioritiesComfortable with daily travel between facilitiesProficient in electronic medical record (EMR) systemsReady to take the next step?
