• Provide therapy services in both SNF and home health environments
• Supervise two assistants as part of patient care delivery
• Utilize Net Health and HCHB software for documentation and patient records
• Meet productivity expectations of 82% for SNF and 95% for home health
• Ensure effective communication and care coordination across settings

Requirements of the Physical Therapist

• Valid Physical Therapist license
• BLS Certification
• Willingness to travel between Grand Island, Hastings, and Kearney as needed
• Flexibility to train and work with new graduates under supervision
