Our providers coordinate patients' care and collaborate across a multidisciplinary team – including physicians, advanced practice providers, care navigators, pharmacists, dieticians, and social workers – to address both immediate and long-term patient needs.Upperline serves patients nationwide, in clinic and at home, delivering expert care in chronic condition management, podiatry, vascular, endocrinology, wound care, primary care, and more.As we continue to expand, we remain committed to making high-quality, coordinated care more accessible and effective—while creating opportunities for our team to make a meaningful impact on the lives of the patients we serve.As we continue to scale Upperline Plus in our regions, Upperline Health is seeking a Medical Director for the Baltimore market to advance clinical excellence, strengthen provider engagement and lead clinical value-based care across the market.This role combines direct patient care, clinical leadership, and VBC program management and partners closely with the central interdisciplinary team, population health and operations teams.With a patient-centered approach the Medical Director will see a small panel of their own patients, provide clinical oversight and hands-on coaching to Upperline Plus' nurse practitioners, identify market opportunities, improve clinical processes, and lead VBC and population health initiatives – all with a focus on optimizing the care model to improve patients' outcomes and quality of life.The position will be based in the Baltimore area. Approximately 30-50% of time will be spent in the clinic setting.Required Experience and SkillsDoctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.)Board certified in Internal Medicine, Family Medicine, Preventative Medicine, or GeriatricsActive license in Maryland and willing to pursue multi-state licensure if needed~ 5 years of outpatient practice experience, inclusive of caring for adult / senior, high-risk, polychronic, clinically complex patient populationsClinically practiced in a value-based care model and/or championed programs to improve quality, patient outcomes and reduce riskClinical oversight or coaching of other providersMust reside in market; be willing and able to provide direct care to a small panel of patients and routinely visit clinics and providers within the marketPreferred Experience and SkillsExposure or experience improving or implementing clinical processes, protocols, or evidence-based guidelinesExperience or expertise in VBC programs (e.g., Transitional Care Management, Complex Care Management, ED Diversion, Advanced Care Planning, High Risk Patient Outreach)Compensation and BenefitsCompensation is commensurate to compensation for similar positions in the region and based on prior training and experienceComprehensive benefit options include medical, dental and vision, 401K, PTO, and parental leaveMedical malpractice, licensure, and boards dues paid annuallyCME expense allotmentJob Type: Full-timeEqual Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
