OVERALL OBJECTIVE: Coordinate, implement, and supervise all nursing and personal care services at CBAS/ADHC Provide skilled nursing care. All policies and procedures of the ADHC/CBAS center and applicable laws and licensing requirements will always be followed.

RESPONSIBLE TO: Program Director
RESPONSIBLE FOR: LVN, CNA’s, and Program Aides.

DUTIES AND RESPONSIBILITIES:

1. Conducts nursing assessments and develops a Person-Centered Plan of Care and is responsible for providing treatments as specified on the IPC and will document the outcome of these services on the same day as provided. The Registered Nurse will also document the outcome of the treatment intervention on the daily treatment flowsheet. The RN will also document progress notes when needed. When indicated, the RN will revise the Person-Centered Plan of Care to ensure appropriate treatment.

2. Monitors vital signs and supervised the LVN/ CNA regarding abnormal readings daily.

3. Conducts Care Coordination with the participant/authorized health agent and primary care physician regarding the participant’s care at the Center. This includes monitoring of vitas, medication management, personal care, and any other treatment intervention within the scope of practice for the Registered Nurse.

4. Provides health counseling to participants and their families on an individual and group basis.

5. Develops and provides health-related training to staff, volunteers, and families.

6. Refers participants and their families to appropriate community health care agencies or facilities, acts as a liaison with such organizations.

7. Responsible for the coordination of personal care activities of all participants including activities of daily living training and continence training. Supervises the program aides in their personal care duties and oversees the staff participating in personal caregiving, while also participating as a personal caregiver.

8. Completes with complete signature and date, quarterly progress notes, and reassessments on all participants. Reassessments may be completed more than every six months due to a change in the participant’s status, a participant’s length of an absence, or following a hospitalization.

9. Collaborates with the Center’s Staff Physician regarding participant skilled nursing needs and will obtain guidance if needed to amend the Person-Centered Plan of Care.

10. Will be responsible for supervising the OT/PT Maintenance Program and the Program Aides carrying out the interventions on the Person-Centered Plan of Care. Will collaborate with the OT and PT Consultants regarding changes in condition that may warrant a reassessment by the appropriate consultants for revisions to the current OT/PT Maintenance Program.

11. Conducts Chart Audits and will report findings to the Program Director. Will also participate in the Utilization Review Committee process.

12. Will be a responsible person in charge of the nutrition and kitchen services when eh Registered Dietician is not present.

13. Responsible for the supervision of all activities related to medications; to include the destruction of drugs, maintenance of drug supplies and mediations, supervision of all drugs stored at the Center, and the record-keeping which list all prescription medications stored overnight.

14. The RN will be responsible for assessing the participant's ability to self-administer medications and will monitoring and provide training to those participants that appear to be good candidates for self- medication training.

15. Maintains the implementation of the Privacy Rights of the participants during nursing treatment interventions and throughout the Program.

16. Supervises the Fall Prevention Policy and collaborates with the OT and PT when revisions might be needed.

MINIMUM QUALIFICATIONS:

1. Knowledge of public health nursing principles, with reference to aging; of physical assessment techniques and care planning methods; health counseling techniques and practices; and community health services and facilities which are available to participants and their families.

2. Demonstrated skills and experience to assess and monitor elderly health care; provide counseling; conduct health education/training programs; maintaining accurate written and oral reports; use sound judgment in emergency situations; deal effectively with families, staff, and community.

3. Satisfactory completion of an approved nursing school with a current license to practice, preferably as a Public Health Nurse.

4. Experience in working with frail and chronically ill elderly persons and in a multidisciplinary team.

5. Ability to assume other roles as needed in the course of program activities.

6. Flexibility, initiative, creativity, and excellent organizational and time management skills.

CONDITIONS OF EMPLOYMENT:

T.B. and physical clearance for reaching, bending, stooping crouching, kneeling, and grasping; have full range of motion; ability to lift 50 lbs; ability to lift and transfer participants; must have current First Aid and CPR; must have a reliable private vehicle, proof of CA driver's license, and provide proof of current vehicle insurance.
