Community Health Advocate & Outreach Specialist
InterCare Community Health Network
InterCare Community Health Network is seeking a Part-Time Community Health Worker to support the Benton Harbor Food Pharmacy Program. You will assist patients, coordinate food distributions, and connect them with resources such as SNAP and WIC.
The role requires building trust with patients, providing education, and collaborating with providers and care managers to improve health outcomes. Bilingual Spanish/English is highly valued; schedule includes Wednesday–Friday with occasional evening
