Care Manager RN Full time Days

Providence

Description Care Manager RN at Providence Holy Cross Medical Center in Mission Hills, CA. This full‑time position involves 8‑hour day shifts. The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient‑centric care management services for at‑risk patients in an acute‑care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is to ensure appropriate healthcare resources are used throughout the continuum so that the care provided is the right care, at the right time, in the right setting. Responsibilities Assess and plan care for at‑risk patients in the acute care setting. Coordinate interdisciplinary care to ensure continuity and quality. Manage and review resource utilization to optimize patient outcomes. Prepare and execute discharge plans in collaboration with care teams. Document interventions and maintain regulatory compliance. Advocate for patients throughout their care journey. Required Qualifications Associate's Degree in Nursing or equivalent upon hire. California Registered Nurse License upon hire. California Fire and Life Safety Card from the National Organization within 30 days of hire. Two (2) years of acute‑care experience in a medical‑surgical or inpatient setting. One (1) year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire. Preferred Qualifications Bachelor's Degree in Nursing or higher within three (3) years of hire. Case management certification (ACM or CCM).
