Hospice RN Case Manager — Compassionate Care Leader

Horizon Hospice of Colorado

Horizon Hospice of Colorado is seeking a skilled Registered Nurse Case Manager to coordinate and deliver hospice care across the Denver Metro area. You will conduct comprehensive nursing assessments, lead an interdisciplinary team, and provide direct patient visits in homes and care settings. This full-time role starts at 0.8 FTE (32 hours per week) with growth to 1.0 FTE (40 hours). Expect a typical caseload of 12-15 patients and travel within the service area, with mileage reimbursement #J-18808-Ljbffr Horizon Hospice of Colorado
